Be sure to RSVP early for the best price and join our Facebook and Yahoo Groups to hear the latest event announcements about all of our 2016-2017 events! 


2016-2017 Participation Form

Please fill out our combined IL Homeschool Teen and Family Activities Form for 2016-2017 which will remain private and be used in an emergency only.
  1. Download and fill out the document from Google Drive.
  2. Save it to your computer (file - print - save to Adobe PDF).
  3. Email us a copy at ILFamilyActivities@gmail.com
  4. Print, sign and turn it in to Diane or Amanda at entry on the day of the event.
  5. Reprint, sign and deliver for each event throughout the year.
